New Student Enrollment

New student enrollment is completed online.  If your child is coming from Murfreesboro City Schools (MCS), you will also need to complete the new student enrollment and upload all required documents.  If you have an older child in Rutherford County Schools (RCS), you will access your parent Skyward account and select the new student enrollment tab.  If not, you will need to go to the Blackman Middle School website, click on the parent tab, and then the registration tab.  There you will select new student enrollment and complete the Skyward Enrollment Portal application.  If your student is already enrolled in an RCS and is transferring to BMS please reach out to Mrs. Todd.  Email: [email protected] Phone: 615-904-3860 ext. 29111

1. Proof of residency (2 are required) - Current Electric/Water/Gas bill showing the parent or guardian's name and service address or a lease for your residence with all the above information listed.

*** IF, as a parent or guardian of a student, you DO NOT have proof of address in your name please visit our front office to pick up an affidavit packet. This packet will need to be completed, notarized, and returned with additional documentation to satisfy proof of residency. All of this must be completed before a student can start school.***

2. Tennessee Department of Health Certificate of Immunization - For students entering from out of state, parents must take their child's immunization records to the Rutherford County Health Department, 100 W. Burton St., Murfreesboro, or 108 David Collins Drive, Smyrna, or contact a local physician to have the immunization record transferred to the Tennessee Certificate of Immunization.

3. Student's Birth Certificate

4. Current proof of custody - Only a custodial parent or legal guardian may register the child. The custodial parent or legal guardian must show proof of guardianship by presenting a copy of custody papers or court order.  This only applies if parents are divorced, separated, or unmarried, but is very important for us to have on file. Whatever documentation we have on file is what we must operate by in terms of contact and/or early dismissal. Please make every effort to submit this documentation if it applies to you.

Suggested Documents

- A copy of your child's most recent report card or withdrawal grades from their previous school. If grades are not available, standardized test scores are also helpful. This is to ensure we enroll your child in the appropriate classes.

- If applicable, a copy of your child's IEP (Individualized Education Plan) or 504 plan.
