2024 - 2025 Football Cheer Tryout Paperwork

 

The following paperwork is REQUIRED for tryouts and due by April 22nd. Your child will NOT be allowed to participate/stay with out its completion.

 

1. 2024-2025 Final Forms (Must be completed and necessary documents uploaded)

     Required Uploaded documents:

          -Proof of Residence (2)

                -Current Utility Bill

                -Bus Zone Verification

          -Physical Examination Form (2 pages): Must be dated on or after March 1st, 2024

                -Physical Examination Form - page 1 of the physical

                -Medical Eligibility Form - page 2 of the physical

          -Birth Certificate

                -State Official Birth Certificate

     Electronic Forms to Fill Out:

          -Contact Information

          -Demographic Information

          -Health History & Medical Profile

          -Consent for Medical Care

          -Transportation Agreement

          -Personal Affidavit of Responsibility

          -Code of Ethics

          -Proof of Residence

          -ImPACT Testing Consent

          -Consent to Disclose Health Information

          -Cardiac Arrest Awareness

          -Parent Concussion Acknowledgement

          -Student Concussion Acknowledgement

 

2. Two(2) Teacher Recommendation forms - Can be turned in to Coach Polston/placed in her mailbox or emailed to [email protected]
